Dr. Donald A. Eberhart grew up in the suburbs of Southern California and earned his Bachelor of Science in Biology, with minors in Psychology and Art History, from the University of California, San Diego. After completing his degree, he took some time to travel across Europe before attending the University of the Pacific School of Dentistry in San Francisco, where he graduated with honors. After spending most of his life in California, Dr. Eberhart made the decision to relocate to Arizona and begin his dental career in the Grand Canyon State.

He earned his Bachelor of Science in Biology from Kansas State University before earning his degree from Missouri School of Dentistry in Kansas City. He also attended an oral surgery externships at the Truman Medical Center in Kansas City, and at Banner Good Samaritan Hospital in Phoenix while in dental school.

Dr. Tobon completed an Advanced Education in General Dentistry (AEGD) residency after graduation in order to elevate his training in advanced oral surgery procedures such as dental implant surgery and endodontics. Dr. Sharon Bader was born in Trinidad, West Indies. In 1988, she earned her Bachelor of Science form the University of Toronto. In 1996, Dr. Bader graduated from the University of Michigan in Ann Arbor MI. She then enhanced her training by attending a residency in Advanced Education in General Dentistry at the University of Michigan.

The front desk receptionist was very kind and efficient. The hygienist did a phenomenal job cleaning my teeth, and after that was done, Dr. Pulkrabek examined my teeth in great detail. I got a clean bill of health! He didn’t try to push unnecessary services or procedures on me, and he didn’t try to make up problems to fix. I did have a discolored filling on my front tooth (from my last dentist), so I asked him about getting it refilled. He told me, “Sure, can you come in tomorrow?” The following day, the appointment took under an hour, and I walked out with a filling that matched the color of my tooth perfectly. I have so many great things to say about Family & Cosmetic Dental in Phoenix, AZ. They’re always very truthful, attentive, and knowledgeable. As soon as you walk in, the receptionist greets you with a smile, puts you at ease, and makes the process of filling out paperwork run so smoothly. Dr. Pulkrabek has done quite a bit of work on me, and I’ve never been in pain because of it. They’ve turned the field of dentistry on its head - I used to dread going to the dentist, but now, I actually get excited when I have an appointment! Dr. Pulkrabek and his team treat me like family, and I refuse to go anywhere else for dental services!

I have been coming here for months and they wow me every time. The office in Phoenix is great! Everything is clean and up to date. My family has been going to Able Dental for about 2 years now, and we truly love it. We have seen multiple dentists and hygienists here, and while they’re all wonderful I have to give Dr. Muscato a particular shout out, she rocks. I was a little nervous when my previous dentist in Mesa sold his business, but after being Dr. Nia’s patient for a couple of years now all my fears have gone away. Dr. Kunthara and the rest of the staff at Simon Dental Care in Mesa are very kind and knowledgeable. A couple years ago I moved back from college where I had a previous dentist in Phoenix. This place let me come in as a transfer patient and was very thorough in their explanation of everything. They went through my treatment plan, asked me what my goals for my teeth were, took molds and casts of my current teeth, and just overall have been very thorough. I did research and I found a lot of options, but ultimately went with this place, and I’m glad I did. You can tell how much these people love their jobs by the high quality of work they do. I’ve recommended this place to my friends and family, trying to convince my family to drive a couple extra miles to come to Dr. Kunthara. She adjusted a wire for me the other day and then she called me later that day to make sure I wasn’t in any pain and that I didn’t have any questions. I’ve never seen any other dentist do that, and so that’s why I wrote this review. They truly have excellent customer service, they’re like no other place I’ve ever been to, and that’s why I have to recommend this place.
